We often associate a cavity with a painful or sensitive tooth. However, a cavity can develop without causing any pain, especially in its early stages.
This is one of the reasons regular dental exams are important: some dental problems can progress quietly before becoming uncomfortable.
Why doesn’t a cavity always hurt?
A cavity usually begins at the surface of the tooth, in the enamel. Because this outer layer does not contain nerves, the early stages can go completely unnoticed.
As the cavity progresses deeper into the dentin and eventually toward the pulp of the tooth, sensitivity to cold, heat, or sweet foods may begin to appear.
Pain is therefore not always the first sign of a cavity.
Cavities between teeth can be especially difficult to detect
Some cavities develop between two teeth, in areas that are difficult to see and less accessible during brushing.
They can progress for some time without any visible changes.
Daily cleaning between the teeth helps reduce plaque buildup in these areas, but a regular dental exam remains important for monitoring areas that are difficult to reach.
How can a cavity be detected if it doesn’t hurt?
A visual examination can identify many dental problems, but some areas require additional diagnostic tools.
Digital dental X-rays can help the dentist identify cavities developing between the teeth.
At Santé Dentaire VMR, NIRI near-infrared imaging technology may also be used to visualize certain interproximal cavities and monitor their progression.
The goal is to detect changes as early as possible rather than waiting for symptoms to appear.
What signs should you watch for?
Even though a cavity can remain silent, certain changes may eventually appear:
- New sensitivity to cold, heat, or sweet foods;
- Pain when chewing;
- A change in the colour of a tooth;
- Food regularly getting stuck in the same area;
- Spontaneous or persistent tooth pain.
When treatment is required, a dental restoration may be used to repair the affected portion of the tooth, depending on the situation.
Don’t wait until it hurts to have your teeth checked
The absence of pain does not necessarily mean that no dental problem is present.
